Each volunteer must have/create an eAYSO account. Please print a copy of the completed volunteer form, sign it, and mail it to: AYSO, 12501 S. Isis Avenue, Hawthorne, CA 90250. Click here.

 

Online Safe Haven Course

Each volunteer must have an eAYSO log-in and approved volunteer form on file. Proceed to Safe Haven certification course only if you have never been Safe Haven certified. Log on to eAYSO to acquire your AYSO ID # (see above). Click here to take the online Safe Haven course .
